Heather Owens was sentenced to three years’ probation and a $500 fine for pleading guilty to one charge of gross patient neglect, a first-degree misdemeanor. She had the following charges dismissed:
• One count of tampering with records, a first-degree misdemeanor
• One count of patient neglect, a second-degree misdemeanor
• One count of forgery, a fifth-degree felony
He co-defendant, Kelly Quillen, is out on a $10,000 bond and is due back in court on August 27, 2026.
Samantha Thompson was a patient at Praxis of the Firelands by Landmark Recovery when she died in August 2023. Her death was a month after three deaths at Praxis of South Bend by Landmark Recovery in July 2023.
