WTHR 13 Investigates put together a special on Landmark Recovery’s Praxis of South Bend by Landmark Recovery facility. Bob Segall interviewed three former patients. The former patients referred to the experience at Praxis as chaos. They said the facility was understaffed. The patients never saw a medical doctor and saw nurse practitioners via zoom. Two patients stated they either never received their medication or it was given to someone else. This is insurance fraud.
Bob attempted to contact Matt Boyle for an interview, but Matt never responded. Nobody from Landmark responded or sent any official statements. Bob drove to Franklin and discovered the 720 Cool Springs Blvd headquarters was utterly empty. Landmark still lists that address on the website. They moved three and a half miles to 133 Holiday Ct # 102, Franklin, TN 37067. They are operating under the new business name Alsos Behavioral Health Sabra LLC. The sign on the door only lists Alsos.
Landmark is attempting to reopen the Bluffton and Carmel locations. There is an application for Bluffton by 1529 W Lancaster St LLC. Carmel’s application to reopen is under 13594 North Meridian Street OPPs LLC. The Boyles sure love registering LLCs.
Bob attempted to ask FSSA for a comment but never heard back. Bob confronted FSSA secretary Dr. Dan Rusyniak in person. Dan refused to speak with him.

I am a current employee at the Columbus Ohio location, they constantly keep clients in there system for weeks after they have left or gone to the hospital, documenting that they are still in facility. There are a bunch of drugs in the facility. They hired a new executive director “jay” who just sits up in his office and doesn’t help his employees at all. The building has black mold all around it, the plumbing sucks. constant leaks all over the building. about 6 months ago a client had seizure an died in front of all of us. he was asking for help, telling nursing that he doesn’t feel good, and they completely ignored him until it was to late. and it was like it never happened. Matt covered up the entire thing. Man I could go on for days about the stuff I’ve seen here the 6 months of my employment. This place needs shut down immediately before someone else DIES!!!!
