It was a matter of time before LR got evicted from another property. This time, it’s LR of Knoxville. I have been told LR is being evicted for not paying rent. Shocking, I know. LR was evicted from Las Vegas and Oklahoma City for not paying rent. They are currently under eviction from Battle Creek. The incoming treatment center doesn’t accept Ambetter insurance. LR now has 38 out of 48 patients on Ambetter insurance.
There are no open beds, but admissions still send female patients. One woman is sleeping on a cot. This is a commercial facility. I am being told that admissions reps are claiming that they don’t see gender when talking to patients. That’s nonsense. If the staff at the facilities are telling you to stop sending female patients, stop sending them. There are no open beds for women until February 27, 2024.
Making this an all-women’s facility might be good until LR is booted out. The men can’t behave around women staff.
These patients were not removed from the facility. One was allowed to complete the program and stay another two and a half weeks.
LR sent Sharon Putnam to deliver the news, but she could not answer questions. She had somebody on the phone as she gave the news. My source tells me she could not answer questions about the transition or when LR would stop admitting patients. While I want to commend LR for addressing the staff, how long has LR known? I ask this because LR is currently hiring a float nurse RN. The job was posted two days ago. I swear Landmark would set up a lemonade stand without knowing they don’t have any lemons. Even the Keystone Cops were more organized. Time will tell which facility is next to fall.
I went to post this, and this comment was waiting to be approved. Matty addressed staff and advised them that LR had lost Battle Creek, Seymour, Norfolk, and one of the Ohio facilities.
It’s been another crazy few weeks for LR, but that’s like saying water is wet. Landmark was evicted in Battle Creek. They sold the building. Another addiction treatment company will be moving in. Landmark has two posted positions for this facility. They have published an open position for a Director of Nursing (DON). The other is a Strategic Account Liaison. Who posts jobs for a facility that’s gone? Landmark, that’s who.
Matt sent out a sad excuse for an apology over the paper checks fiasco. While Matty stated he screwed up over the latest checks being delayed, he chose to point the finger at his controller and payroll manager. Matty claims they thought they were saving money by sending the checks via two-day delivery instead of being sent overnight. Listen here, Matty, it’s not just the fact that checks are late, your employees are struggling to cash, or when they deposit the checks, get them to clear. You don’t know about the stress and hardship this puts people through.
The entire marketing department is gone as of Friday, February 16, 2024. Landmark is hiring an outside company to handle the marketing. How long until I start not to get paid? So why is Landmark still looking to hire a marketing manager? Does the right hand even know what the left hand is doing?
I was informed that the Assistant Executive Director at Denver was offering Venmo Patient Engagement Specialists $50 to work late or stay and do intakes.
There is an allegation that LR terminated the Director of Nursing at Medicaid Facilitry of Louisville by LR this week. Here is the message in detail:
“Hi Jason. I don’t want to post it attached to my name, but yesterday the DON at Medicaid Facilitry of Louisville was fired. Evidently the nurses had been keeping the medication of patients who had left (meds they brought in) and had been using them as house stock giving them to others. Jessica Tate is said to have known about it and told the DON not to do this but it had been going on for many months. Also, they are taking patients and putting Valium and certain meds on their admission orders, but this location is no longer receiving Valium so idk what they are giving them or why it’s on the orders. They announced yesterday there will also be no more Suboxone or Gabapentin either. Melatonin has not been coming for quite some time and nursing staff or other staff have actually been buying the Melatonin for the facility”
I have heard of other facilities not having melatonin.

Update February 17, 2024, at 4:46 PM, PST. Landmark was served an eviction notice, and just like they did with Las Vegas and Oklahoma City, they are hiring and accepting patients. The staff hasn’t been informed. I love the LR Sucks trolls. They are not the sharpest tools in the shed. They bring a butter knife to a gunfight. Today, I was removed as admin from A Collective to Hold LR Responsible. It was over a disagreement over a member being removed and banned. I defended them and attempted to get them reinstated. I don’t want to get into the weeds, but I was overruled and removed as admin. My posts need admin approval. I don’t want to go into the drama, but I was repeatedly gaslighted today. I attempted to call the admin and make light of them, stating they needed to go to bed, to see their icon show up as seeing my message. Every attempt to apologize and right the ship was ignored, while I was told I was not honoring boundaries.
I started a new group. The Collective will die. I will let them run the group however they deem fit. Come and join the new group.
I was logged off of Facebook twice within a week. The first time was after I posted the link to The Collective. Since then, we have gained 50+ members. Yesterday, I was logged off of Facebook for the second time. I don’t know if they are trying to reset my password, but I can log back in. Booting off of Facebook will not stop or silence me. I received some hate comments when I posted how former employees can get access to Paycom for their W2s. That post triggered folks as it received two troll comments.
I messed up and didn’t use they/them pronouns without knowing Leighton’s gender. That’s my bad. I usually type my content in Microsoft Notepad and edit it in Grammarly. I didn’t because The Collective kept receiving duplicate posts asking how to get the W2s. While we don’t mind being HR, we must address other things. We are volunteers with full-time jobs and responsibilities. I adopted Ms. Tina Brown two weeks ago today. Happy gotcha day, Ms Tina. I started a full-time role on February 1st. Sorry LR trolls, I won’t tell anybody where I work. I learned how to protect myself and my employers while I help people.

People thank me for creating this website. People thought they had they were crazy as the issues they faced were isolated to their facility. This website helped people to see the truth. The issues they were experiencing were happening at other facilities. I told the investigator at Indiana’s Attorney General’s office where to find the dead bodies. FYI, there was a DEA agent on the call, too. I am compiling a list of people to be interviewed. The Department of Justice, the DEA, and the Attorney General’s Office are all investigating LR. I and others have been interviewed by agents at the FBI.
I launched this website to document the news stories. I started getting emails from people with inside information. A few stories broke me. I have cried being a consumer advocate; three times were from the shit I heard about Landmark. I broke down sobbing while on the phone with somebody as I read a message. Men are emotional, we have joy, laugher, and anger. So, to the haters, you can’t stop me. Your hate fulls me. I know that I am upsetting you and living rent-free in your heads.
I am here to protect Landmark employees and patients. I won’t stop, as people’s health and well-being are on the line.

I received a disturbing email regarding Medicaid Facilitry of Norfolk by LR. Here is the entire unedited email.
“I am hesitant about doing this out of fear of the possible repercussions. However things have reached a breaking point for me now. The first floor at the PORF location had flooded with a sewage back up twice in one weekend. During this time the PES department was grossly understaffed. Second shift was a skeleton crew and third shift only had one PES and thankfully the nursing department was fully staffed. The manager that was on call that weekend wasn’t answering staff onsite calls until almost 6 AM. Once the morning came and they thought it was fixed, come the end of the second shift the exact same issue happened again. And yet again third shift PES department only had one PES show up to work all three floor and 60 patients! On call management again didn’t answer until 6 AM again and it took hours for any plumbers to arrive onsite to try to fix the problem. The admissions department was still sending new intakes knowing what was happening. Now over a month later, the entire first floor is under construction. It is completely shut down and now 60 patients are on two floors. They’ve limited females to 18.
Patients are definitely having sexual relations with each other. At least one staff member has been fired for inappropriate conduct and poor boundaries with patients. Staff that was working in the intake department were not conducting proper luggage/skin searches which was permitting contraband into the facility. Documentation of patients property was not being done properly. Many times at time of discharge when patients were to sign for their property only to discover a phone was missing or other high value property.
If a patient wants to leave Against Staff Advice (ASA) at one time as long as a staff member onsite had safe code access the patient would be able to leave with all their property. Then recently it changed to no patient will receive their property from the safe after 5:30 PM. And if a patient is leaving ASA after 5:30 PM or on weekends that patient would need to figure out their own transportation from the facility.
First fights break out and many times there is no consequences for those that fight. Nobody is administratively discharged or possibly even placed on a behavioral intervention. Patients are having clothes, snacks and other property stolen while in groups and nothing is investigated or done about it. Patients many times will threaten to either physically harm or sexually assault staff with no consequences for such threats. Patients are constantly punching holes in the walls, breaking locks and doorknobs in the facility and again there are no consequences for destruction of property.
Dietary has switched management and since this has happened the quality and quantity of food has decreased significantly. Many of the patients have complained and submitted grievances as a result of this. The patients used to be able to get seconds and now (before first floor construction) they’re lucky to get a decent first serving. And with the first floor construction, lunch and dinner are being catered.
Home office isn’t even keeping with their own policies that were put in place. Many staff members will discuss amongst themselves that it is evident that the company is more concerned with heads in the beds to make a profit than they are the quality of their treatment.
The very last straw was the very night before payday a communication was sent out stating we would not be getting direct deposit but paper checks and would need to come in person to pick them up! No explanation or anything. Please do not list my name as I need to keep my job until I am able to find another one.”
I wanted to address the improper patient-to-staff ratios. Landmark got into trouble for being understaffed. One PES for 60 patients is not only unsafe, it’s dangerous. The Executive Director is supposed to notify the state when they are short-staffed. A surprise inspection is in order. The authorities would love to see the porta potties in the parking lot. Last Friday, a patient was found unresponsive inside one of the porta-potties. Staff was instructed to search them.
The first floor is under construction, and the 60 patients are on the two floors. It’s no wonder why patients are acting out. They are barely getting enough food. Men are entering women’s rooms. Patients are stealing people’s property. Patients complain that their personal effects are missing when they are discharged.
I hope the state revokes Landmark’s license and kicks them out of Virginia.
It has been a whirlwind of a week. Last Saturday, I adopted Tina. On Thursday, I started a new full-time job. The next thing I knew, Landmark News exploded. LR has been putting the properties into separate LLCs thanks to a great source. This was discovered because contractors have liens on two Ohio facilities for $664K. I was also advised that the Boyle’s placed their homes into private trusts. I don’t know when they did this. I am knocking out the blog post about the conditions at Norfolk. I got distracted by Landmark canceling the direct deposits for the employees at the facilities. This has generated a lot of texts, emails, and DMs. We have admitted 21 new people into The Collective. I am exhausted.

While I don’t know what happened, an anonymous source claims LR was forced to cancel the direct deposit on Wednesday and issue paper checks. Several people were freaking out about this. We had four new members join The Collective Facebook Group yesterday. We usually get a handful of new members a month. Several people and the admins have been following this. We were curious to know if the checks would bounce. While we are still determining if the checks will bounce, no check cashing facilities will accept them. They need help getting ahold of somebody at Landmark to verify the checks or employee’s status. The banks want to hold the checks for up to two weeks.
When this was brought up in Teams Chat, Tia Conard didn’t understand the issue’s magnitude. Well, Tia, people have rent and bills to pay. Well, Tia, people’s fears have now been confirmed.
The admins and members of the Collective feel bad for everyone who received a paper check. Please contact your local news stations and tell them your story. If you make enough noise, Landmark will get you paid sooner rather than later. If you need assistance, please reach out to me.

One facility has advised me that Landmark will issue paper checks until they find a new payroll vendor. Some people could get their funds released when they contacted their bank and advised that this was a payroll check.
Today, I was alerted to this missing Google review regarding a former patient’s stay at LR of Knoxville. Google didn’t allow the review to be published. At first, I thought it was because the former patient posted her email address. Either the nanny bot found words that triggered the review to be flagged, or the aggressive algorithm prevented the review from being published.

“First of all, I was sober, with the help of God and AA alone, for 4 years – the whole time working in the recovery field myself at Addiction Campuses and Foundations Recovery- and relapsed in 2021 after my boyfriend had a stroke and my sister died, all within two months of each other. I VOLUNTARILY put myself into LR in Seymour, TN December 17, 2022, choosing to do so because I was struggling. From the day I admitted, noone was expecting me and they treated me like a nuisance during intake. My therapist quit the first week I was there. She told me before she left that Landmark had told the therapists there to end the call if we wanted to leave. After she left, I was never assigned another therapist. The sad thing about that is, you couldn’t make phone calls without one. Anyone I asked said they would lose their job if they let me use the phone, and they couldn’t tell me who to go to get reassigned. I went three weeks without speaking to my then 11 and 12 year old; even over Christmas. I cried myself to sleep that night, and they were upset with me when I came home. It did a number on our relationship and that’s something I’m still having to earn back. I was never assigned a psychiatrist and was completely detoxed off of my prescribed medication. They tried to force me to take detox meds ten days against my will; told me I’d have to stay in my room if I declined my vitals. Facilities can charge more for detox vs residential, so they would deny me drugs like Clonidine because they are non-controlled which means insurance pays less and moves me sooner to residential. I would have to build my anxiety so high that my blood pressure would reach a point that they would give it to me. Everyone did ten days. Didn’t matter if they needed it. I never received any of my calls or mail from home. The large workbooks we were supposed to receive we were charged for and never received. They mixed male and females in a cabin with absolutely no system in determining who should qualify. One put his hand on a girl’s crotch so they moved him downstairs. Men came into our rooms without knocking. They took our towels without us knowing and redistributed them. The staff cussed at us. We were locked inside at ALL times and were never allowed to leave campus or go outdoors. It was cramped and it smelled horrible. They were short on everything- food, toliet paper, towels because they were just packing us in. Later they took cots into rooms so they could stuff more in.I had to wash my clothes three times to get the smell out. My mother will attest to this. Detox patients and Residential patients were mixed from day one, and we had thirty minute room checks every hour detox or not, every night…full on flashlight in the face, door slammed. If you AMAd they kicked you out immediately and would call the cops if you were on campus, then they made you walk LITERALLY in the snow in freezing weather down a large hill. and were members of the staff and sleeping together; they were extremely open about it and treated us like dogs. A male patient, T fell one weekend and the staff would not let him leave campus to be treated….As in actually restricted him. He ended up having to have surgery on his leg because he went untreated. E was promised a case worker would be assigned her case and that someone would contact her job for the state as a correctional officer; she was fired because it never happened. S had a horrible eating disorder that should have been prioritized over her alcoholism. T had her suboxone stolen from the nurse’s office because it was left unlocked; and while on that subject, the nurses station was directly beside the commons area and you could hear everything the nurse said (HIPPA?) You had freaking employees showing slides that say “there is 100 percent chance of relapse without aftercare” implying LANDMARKS aftercare. TOTALLY false information- I STILL HAVE THE HANDOUTS!”
I have heard the above allegations countless times. Patients are not able to use the phone without somebody listening in. Of course, LR will say this is to prevent drugs from being requested, but this former patient claims it was to prevent them from leaving Against Staff Advise (ASA). ASA patients would be forced to leave the property immediately without transportation to get home. I am shocked that there weren’t any deaths like Bluffton.
Her patient’s therapist quit the first week she was there. She was not assigned another therapist and was unable to call her kids for Christmas. She didn’t receive all of her phone calls or mail. She was never given a psychiatrist and had to detox on her own. There are allegations of attempting to force her to take detox medication ten days after being at Landmark. She suggests it’s insurance fraud.
I have been communicating with her, and she has advised me that staff members were sleeping with each other. They were open about patients who were also sleeping with each other. She also told me that suboxone was stolen. This was captured on video. Was this reported to the proper authorities? I doubt it.
This review claims Landmark doesn’t care about patients, their well-being, or their safety. The review stresses that it’s about Heads in Beds and making money.
I am being told that the newly opened Medicaid Facilitry of Columbus by LR could be doing better. The 192-bed all-male facility opened on October 13th, 2023. Several former LR employees were concerned with a facility this size opening after what transpired at Mishawaka. I have several sources at this facility, and the news I hear is disturbing. Patient Engagement Specialists (PES) work 16-hour days. The facility is understaffed. I am being told that all employees must punch out for lunch, even though they can’t take a break.
On January 3rd, 2024, there a patient overdosed and was taken to the hospital. This patient has previously left ASA and returned three days later. The patient was taken to the hospital and was saved. Before the patient’s overdosing, LR was advised that Fentayly had made its way into the facility. Nothing was done with this information. Patients are cheeking medication placing it in drinks, and passing it around. Several female staff members feel unsafe. One patient has exposed himself and told a nurse that he wishes to rape her.
Patients who should be discharged are not, while others leave ASA daily. LR allowed a trans female to be admitted. They should never have been admitted to an all-male facility. A nurse made a transphobic remark to them. They eventually were discharged after contracting COVID-19 and refusing to follow protocol. The nurse is still employed.
Drew Mack is the Executive Director. She is leaving Landmark at the end of February. She has been a longtime loyalist. She has her LinkedIn set to open to work. I am shocked that she has had enough. She has been a longtime supporter of Landmark. She was switched from Indianapolis to the home office. She will not be missed, and I haven’t heard pleasant things about her.
I am being told that patients are still being held against their will, and staff refuse to give them their personal belongings when they attempt to ASA. Patients are given phones with dead batteries, so they can’t call anybody. This policy was supposed to end in March 2023 after a patient died leaving the Bluffton facility. Yet another one of Matt Boyle’s lies. This facility is imploding, and it’s a matter of time before something horrible happens. I hope the states shut them down ASAP.
Update 1/20/2024
I have been advised by several sources that a pipe burst this past week. There were several days when the water was turned off. Once the pipe was fixed, a couple more burst. The kitchen needs a deep cleaning before it can be reopened. They are currently bringing in meals. They are presently running generators to heat the kitchen and dining room. The heating has yet to work since they opened in October. I feel sorry for the staff and patients at this facility.
