Clinical Liason Kyle Brewer was on Good Morning Arkansas talking about how wonderful Praxis of Little Rock by Landmark Recovery is. Kyle showed off photos of the facility before its opening. I seriously doubt it is still in the same condition.
I emailed KATV and asked them to take Kyle up on his offer to tour the facility. I’d love for them to look inside and speak with patients and staff. I wonder if Kyle has toured the facility or knows what happens inside. He’s outspoken about addiction treatment and helping people recover. He would quit if he knew a fraction of what I know.
Here are the findings: 1. Failure to Conduct Required Patient Observations:
Landmark Recovery’s Patient Observations Corporate Policy requires staff to conduct “Q Checks” at least every 30 minutes for the first four days of each treatment episode.
A review of security footage confirmed that the Licensed Practical Nurse (LPN) and Patient Engagement Specialist (PES) on duty failed to conduct the required observations for Ms. Knight prior to and after the assault.
The PES acknowledged entering Q30 observation entries into the medical record system without conducting the required visual checks of the patient, as mandated by the facility’s internal policy. This practice constitutes a violation of established observation and documentation protocols.
Ms. Conrad, Ms. Hibbs, and Ms. Nelson were aware of this policy violation at the time of the site visit.
Ms. Conrad stated that Mr. Ochs, the PES, was terminated as a result. However, Mr. Ochs contested this statement and said that he voluntarily resigned.
2. Failure to Enforce Restricted Access to Patient Living Quarters:
Facility policy states that patients may not enter other patients’ living quarters.
Security footage showed multiple instances of patients entering other rooms, including Ms. Hensley entering Ms. Knight’s room multiple times.
3. Failure to Provide Appropriate Medical Response for Withdrawal Management:
No clear documentation was found indicating that the prescriber had been notified of Ms. Hensley’s inability to receive her prescribed medication.
According to 908 KAR 1:372, Section 2, protocols must be developed and supported by a physician knowledgeable in addiction medicine to ensure appropriate care when a patient’s condition deteriorates.
A review of Ms. Hensley’s medical records confirmed she was prescribed Diazepam upon re-admission on 01/22/25, as she had previously been prescribed Diazepam at The Brook.
A treatment team note dated 01/29/25 stated, “Angela is currently prescribed a Diazepam taper but has not been able to receive this medication due to having a pharmacy lock from insurance.”
Medical records noted Ms. Hensley experienced withdrawal symptoms, including restlessness, runny nose, and anxiety, and continued to experience auditory hallucinations.
Not only does it sound like this death was preventable, but these are the same issues I have heard about at other facilities. It sounds like Landmark has a lawsuit on its hands. They were dinged for not performing patient checks and falsely stating that checks were completed. Ms. Henlsey wasn’t given her prescribed medication. Sounds like an insanity defense.
Here is how Landmark Recovery plans to correct the findings:
Performing daily checks of surveillance cameras and requiring an administrator at each facility to visually check camera records to confirm that patient observations were completed.
The immediate termination of staff members if they are found to be falsifying patient observations.
Requiring patients to sign an additional consent form upon arrival to indicate they agree not to enter other patients’ rooms and termination from the treatment program for a second violation of the policy.
Assigning staff to monitor residential hallways for loitering patients.
Assigning an administrator to review camera footage weekly for patients who enter rooms inappropriately to ensure proper compliance and monitoring.
Requiring the director of nursing to escalate details about the inability to receive patient medications, sending that information to the compliance department and attending provider (Landmark claimed Hensley did receive the medication she needed while in treatment.)
I am calling bullshit here. Time and time again, Landmark has refused to make the necessary changes. This has been a recurring issue since day one. I don’t see it getting any better. How many more people need to die inside a Landmark facility?
When WTHR reporter asked Matt Boyle for a statement after the findings, Matt lashed out. Matt texted back with, “Do you ever fucking stop? Seriously I hope you burn in hell.” Matt emailed the following statement: “As it relates to the confidential findings you somehow claim to have obtained regarding the sentinel event at our facility in Kentucky, I have attached evidence that our plan of correction was accepted by the state and our license remains in good standing. It is standard practice for regulatory bodies to review licensed facilities. Virtually all reviews reveal opportunities for performance improvement. Responding to opportunities with a plan of correction is a routine and important part of the quality improvement process in behavioral healthcare. I will further remind you that this particular facility operated for over eight years and served over 10,000 patients without a single sentinel event. I believe the state’s decision not to take action against our license reflects this reality.” Wow Matty, way to play the dicktim.
Today, I was alerted that Landmark Recovery will participate in the Spring into Recovery Open House. The event will occur on Friday, April 25, 2025, in Hot Springs, Arkansas. I am being told things are worsening at Landmark Recovery of Denver. The facility currently operates without an Executive Director and a Director of Nursing. Corporate is training the Qualified Medication Administration Personnel (QMAPs) and planning on firing the entire nursing staff. Landmark readmitted a patient who had sex with an employee and got her pregnant. There is no steady cleaning staff, and patients are handling the cleaning. Patients with diabetes have to furnish their snacks. Patients are being kept longer because Landmark is struggling to set up housing.
Landmark Recovery closed the Praxis of Louisville facility after a patient died. Angela Hensley has been charged with murder. She is pending the mental examination results to determine if she is fit to stand trial.
Landmark let all employees go, minus the Executive Director (ED). This is surprising since the ED is typically fired after a patient dies. The ED was transferred to another facility, which means another ED was let go. Matt didn’t personally handle the firings; the RVPO handled them.
Where is Matt Boyle?
Matt oversees the two Ohio facilities. I am told that the Praxis of Columbus by Landmark Recovery is in disarray. Any patients speaking up are being transferred to Praxis of Cleveland by Landmark Recovery. The RVPO is working on opening the Colorado Springs facility.
I am told that two employees were recently fired at Landmark Recovery of Indianapolis. It’s been alleged that the two employees were having a sexual relationship with a female patient. From my source, the two employees were exchanging sexual photos and text messages with the patient. One terminated employee allowed the patient to stay at their residence. I have been provided the names of both terminated employees. I am not releasing them or their roles at Landmark for now.
Louisville police are currently investigating a patient death at the Praxis of Louisville by Landmark Recovery facility on Sunday, February 9th, 2025. The news article states the female patient was deceased when the police arrived. Matt Boyle said, “The patient had a pulse and vital signs when leaving our facility.” Matt also stated, “Neither patient is currently in our care.” Matt is unsure if the other woman will be allowed back at Praxis. Matt said in a phone interview, “Not every incident can be learned from.” I am shocked Matt replied.
This patient was under Landmark’s care. Another family lost a loved one.
Update February 11, 2025
A female patient was arrested for murder. It’s alleged that the deceased wanted to die, so the woman killed her. The patient was found unresponsive on the floor of her room with an with an apparent laceration to her head. The victim was found unresponsive just before 9 p.m. Police showed up after 10:00 PM. It seems that MattBoyle’s statement is a lie!!!

Louisville police have upgraded the charges from attempted murder to murder.
According to her arrest citation, surveillance video shows Hensley entering the victim’s room Sunday at Praxis of Louisville by Landmark Recovery in the Hazelwood neighborhood. Louisville Metro Police investigators said Hensley is initially seen walking into the victim’s room, and the victim could be seen standing.
About four minutes later, according to the arrest citation, Hensley is seen leaving the room, and the victim can be seen lying on the floor. She then reenters the room multiple additional times and never told staff anything about the victim’s condition.
“She said she wanted to die so I killed her,” Hensley told LMPD detectives, according to the arrest citation
Hensley added that she reentered the room several times because “she wasn’t dead yet” and “I f****** finished killing her.”
I have several questions. Why did Matt Boyle lie about the status of the deceased patient, and why did the Louisville police not admittedly arrest Ms. Hensley for murder?

Surveillance video shows Hensley entering the patient’s room at 8:54 PM and shutting the door. She left the room at 8:58 PM. The patient was discovered at 9:57 PM.

Angela was ordered to undergo a mental health evaluation.
WTHR 13 Investigates put together a special on Landmark Recovery’s Praxis of South Bend by Landmark Recovery facility. Bob Segall interviewed three former patients. The former patients referred to the experience at Praxis as chaos. They said the facility was understaffed. The patients never saw a medical doctor and saw nurse practitioners via zoom. Two patients stated they either never received their medication or it was given to someone else. This is insurance fraud.
Bob attempted to contact Matt Boyle for an interview, but Matt never responded. Nobody from Landmark responded or sent any official statements. Bob drove to Franklin and discovered the 720 Cool Springs Blvd headquarters was utterly empty. Landmark still lists that address on the website. They moved three and a half miles to 133 Holiday Ct # 102, Franklin, TN 37067. They are operating under the new business name Alsos Behavioral Health Sabra LLC. The sign on the door only lists Alsos.
Landmark is attempting to reopen the Bluffton and Carmel locations. There is an application for Bluffton by 1529 W Lancaster St LLC. Carmel’s application to reopen is under 13594 North Meridian Street OPPs LLC. The Boyles sure love registering LLCs.
Bob attempted to ask FSSA for a comment but never heard back. Bob confronted FSSA secretary Dr. Dan Rusyniak in person. Dan refused to speak with him.

I had another reach out about issues inside of Landmark Recovery of Denver. Current employee posted a comment on my Landmark Recovery of Denver is out-of-control post claiming what I posted was not valid. Then why do I have another person contacting me advising of ongoing issues? I was told there are unsafe working conditions. Individual therapists and group therapists see up to 40 patients after being hired without training or supervision. The facility didn’t have enough food over the holidays. The printers don’t work and run out of toner and paper. Patients are forced to attend group sessions from 10:00 AM to 5:00 PM, with a one-hour lunch break and ten-minute smoke breaks each hour. Drugs are making its way in. Patients and staff are stealing from other patients. If this is cleaning house, I don’t want to know how much worse it was.
Praxis of Columbus by Landmark Recovery appears to be taking after Landmark Recovery of Denver. There is a mass exodus of the staff. Some staff have already left, while others have put in their notice. Staff quit after Landmark Recovery changed insurance providers, and premiums increased. Once several staff members quit, others felt the company was unstable,e and they jumped ship.
Did Landmark change insurance providers because they didn’t pay the last one to cut costs or both? Will employees at other facilities follow suit and quit?
